29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) At the job site, on or about and at times preceding to September 7, 2017, extension ladder to second level (9 feet), only extended 1.5 feet beyond the roof line.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support). (a) Job site, front of house, on or about and at times proceeding to September 7, 2017, the extension ladder used to access the second story level was maintained at an extended/wide angle.
