University of Saint Mary of the Lake: On or about September 7, 2017, the employer failed to record an injury within 7 calendar days of receiving information that a recordable injury or illness had occurred. a) On or about June 22, 2017, a recordable injury occurred when a grounds keeping employee was hospitalized after sustaining a deep laceration to the left forearm and cut on the upper lip after falling from a ladder while using a chainsaw to trim a tree. The employee's injuries resulted in over 55 days away from work.
