29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    a) Mogas Tank No. 9-181-1 USNS PFC Dewayne T. Williams:  After employees had received electrical shocks while descending into the tank it was discovered that the 120 volt electrical cords entering the tank had not been effectively inspected. One cord was found to have a section of the insulation removed, exposing the bare wire underneath; another cord was found to have a missing ground pin on the plug-end and outer insulation on that same cord was pulling away from the plug-end.    ABATEMENT DOCUMENTATION IS REQUIRED
