1910.30(a)(1) (a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart. Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. A) Vessel Laramie, on or about 4/11/2023, and or times prior to, the employer exposed employees to a fall hazard in that the stair rail system leading to the gangway did not have a top rail on the left open side going up and on a section of the right open side going up.

29 CFR 1910.106(d)(7)(i):Suitable fire control devices, such as small hose or portable fire extinguishers, were not available at locations where flammable liquids were stored: A) Lay Down Area, on or about 4/12/2023, the employer exposed employees to a fire hazard in that there was no fire extinguisher available in the above ground diesel fuel tank storage area.
