29 CFR 1910.140(c)(22): Personal fall protection systems were not worn with the attachment point of the body harness located in the center of the employee's back near shoulder level. The attachment point was not located in the pre-sternal position if the free fall distance is limited to 2 feet (0.6 m) or less. a. On or about September 5, 2017, at the above addressed jobsite, an employee tied a rope to the chest strap of his body harness to be used as part of a personal fall arrest system. b. On or about September 5, 2017, at the above addressed jobsite, the D-ring of an employee's body harness was positioned below shoulder level.

29 CFR 1910.140(d)(2)(ii): Personal fall arrest systems were rigged in such a manner that the employee could free fall more than 6 feet (1.8 m) or contact a lower level. A free fall may be more than 6 feet (1.8 m) provided the employer can demonstrate the manufacturer designed the system to allow a free fall of more than 6 feet and tested the system to ensure a maximum arresting force of 1,800 pounds (8 kN) is not exceeded. a. On or about September 5, 2017, at the above addressed jobsite, employees worked from the edge of a building and were utilizing lifelines that allowed free falls of up to approximately 30 feet.

29 CFR 1910.30(a)(1): (a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart. Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. a. On or about September 5, 2017, at the above addressed, employees used non shock absorbing life lines with up to 30 feet of slack tied directly to their body harnesses. In the event of a fall, the forces imposed upon the employees would exceed 1,800 pounds.
