29 CFR 1910.176(b): Storage of material created a hazard (e.g., by sliding or collapsing): a) Betenbender machine area, 1711 Route 588, Fombell, PA 16123: On or about July 27, 2017, a spare die that was stored on top of the Betenbender hydraulic press brake (model number 10-120T, serial number 73098) was not secured to keep if from falling. Employees operating the Betenbender hydraulic press brake were exposed to struck-by and crushed-by hazards.
