Zero exposure was assumed for the unsampled 241 minutes of the workshift.     (b) Goodhart Sons Inc. - An employee working in the fabrication department performing welding and grinding operations was exposed to a noise level of 89.1 dBA as an 8 hour time weighted average during one workshift on September 13, 2017 over a 268 minute sampling period. Zero exposure was assumed for the unsampled 212 minutes of the workshift.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.    ------------------------------------------------------------------------------------------------------------------------------------  Per ISA on January 4, 2018 - the following changes were made to the AVD      (a) Goodhart Sons Inc. - An employee working at the abrasive blasting operation was exposed to a noise level of 89.6 dBA as an 8 hour time weighted average during one workshift on September 13, 2017 over a 239 minute sampling period. Zero exposure was assumed for the unsampled 241 minutes of the workshift. The hearing conservation program was deficient in that the audiograms were not conducted annually and all employees were not trained annually.
