29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    (a) Goodhart Sons Inc. - An employee working at the abrasive blasting operation was exposed to a noise level of 89.6 dBA as an 8 hour time weighted average during one workshift on September 13, 2017 over a 239 minute sampling period. Zero exposure was assumed for the unsampled 241 minutes of the workshift.     (b) Goodhart Sons Inc. - An employee working in the fabrication department performing welding and grinding operations was exposed to a noise level of 89.1 dBA as an 8 hour time weighted average during one workshift on September 13, 2017 over a 268 minute sampling period. Zero exposure was assumed for the unsampled 212 minutes of the workshift.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.    ------------------------------------------------------------------------------------------------------------------------------------  Per ISA on January 4, 2018 - the following changes were made to the AVD      (a) Goodhart Sons Inc. - An employee working at the abrasive blasting operation was exposed to a noise level of 89.6 dBA as an 8 hour time weighted average during one workshift on September 13, 2017 over a 239 minute sampling period. Zero exposure was assumed for the unsampled 241 minutes of the workshift. The hearing conservation program was deficient in that the audiograms were not conducted annually and all employees were not trained annually.    (b) Goodhart Sons Inc. - An employee working in the fabrication department performing welding and grinding operations was exposed to a noise level of 89.1 dBA as an 8 hour time weighted average during one workshift on September 13, 2017 over a 268 minute sampling period. Zero exposure was assumed for the unsampled 212 minutes of the workshift. 29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    (a) Goodhart Sons Inc. - The employer's written respiratory protection program did not include procedures for inspecting respirators and procedures to ensure adequate air quality, quantity, and flow of breathing air for atmosphere-supplying respirators, on or about September 13, 2017.    Abatement certification required within 10 days after abatement date. 29 CFR 1910.134(i)(5)(iv): The employer did not ensure that the person authorized to perform the necessary sorbent bed and filter changes filled out a tag indicating by the signature and date that maintenance changes were performed:  (a) Goodhart Sons Inc. - The employer did not ensure that the required filter changes were completed on the Bullard Free-Air Pump system by filling out a tag that indicated the signature and date that the maintenance had been performed, on or about September 8, 2017.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
