OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to amputation hazards:    a. On or about September 7, 2017 employees were operating the FAMG Air Press Machine in the Kick Press Area and utilizing an actuation pedal that was not equipped with a guard over the pedal to prevent accidental actuation, thereby exposing employees to amputation hazards.       Among other methods, one feasible and acceptable method to correct this hazard is to follow the American National Standard for Machines (ANSI) B11.2-2013, Safety Requirements for Hydraulic and Pneumatic Power Presses, follow the guidelines stated in section 6.4.3.9.4 Foot control. "Foot-operated controls, if provided, shall be protected to prevent actuation from falling or moving objects, or from unintended actuation by inadvertent stepping onto the foot control."

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:    a. On or about September 1, 2017, employees were operating Air Press Machines in the Kick Press area and were not provided with energy control training, thereby exposing employees to amputation hazards.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, were not guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    a. On or about September 1, 2017, employees were operating the FAMG Air Press Machine in the Kick Press Area and the machine was not equipped with proper guarding to protect employees from hazards created by the point of operation, thereby exposing employees to amputation hazards.
