29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)   On or about August 18, 2017:  A TGR employee conducting mortar mixing operations at 761 Baylor Circle, Mathis Place at Pembrooke Subdivision in McDonough, Georgia was exposed to the hazards of silica where they were not provided with effective information and training in the chemicals on the worksite.

29 CFR 1910.134(d)(1)(ii): The employer did not select and use a NIOSH-certified respirator in compliance with the conditions of its certification: (Construction Reference 1926.103)(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION/CONDITION) (DESCRIBE HAZARD)   On or about August 18, 2018: A TGR employee conducting mortar mixing operations at 761 Baylor Circle, Mathis Place at Pembrooke Subdivision in McDonough, Georgia was exposed to silica dust.  The employee was not wearing a NIOSH certified dust mask.
