29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: On or about September 6, 2017 at a worksite located at 44 East Federal St. Youngstown, OH, during commercial construction activities, the employer did not ensure employees and subcontractor employees were provided a stairway or ladder for a change in elevation greater than 19 inches. Employees and subcontractor employees were accessing and egressing different floor levels with a buck hoist and many areas did not have proper access/egress.
