29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about 8 September 2017, and at times prior thereto, workers were performing roofing activities at an apartment complex with multiple buildings without hard hats, exposing the workers to the hazard of being struck by falling materials, tools or debris while on the ground.

29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles. On or about 8 September 2017, and at times prior thereto, workers were performing roofing activities, to include the use of pneumatic nail guns, without wearing eye protection, exposing the workers to the hazard of being struck by flying particles or objects.

On or about 8 September 2017, and at times prior thereto, workers were performing roofing activities on a 3-story roof and a 1-story roof of an apartment complex without fall protection, exposing the workers to a fall hazard of up to approximately 30 feet 6 inches.
