29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a) On or about September 6, 2017, employees installing a new roof to a single family residential home located at 5925 Folsum Place, McKinney, TX.  At least two (2) employees were working from the ground doing various work operations and did not have on a hard hat.  The employees were exposed to head injuries where overhead hazard exists.

29 CFR 1926.102 (a)(1):  Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  (a) On or about September 6, 2017, employees installing a new roof to a single family residential home located at 5925 Folsum Place, McKinney, TX.  Employees were performing roofing operations while using nail guns. Employees were not wearing safety glasses and were exposed to eye injuries.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k) developed and implemented:   (a) On or about September 6, 2017, employees installing a new roof to a single family residential home located at 5925 Folsum Place, McKinney, TX were exposed to fall hazards and were not protected by the use of a personal fall arrest system or safety net system.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:   (a)  Employees were installing roofing tiles on a new residential home located at 5925 Folsum Place, McKinney, TX and were using a 28-feet extension ladder on the front side of the home that did not extend 3-feet above the landing of the roof were exposed to a fall hazard.
