29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: a) On or about the week of August 21st through 25th, 2017 and times prior thereto, located at Naval Weapons Station Cheatham Annex; C Street Warehouse #12, Williamsburg, VA 23185, Condition: Employee(s) prior to working in a confined space did not have access to documented measurements of atmospheric monitoring on any confined space entry-permit prior to entering the space. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146, Permit required confined spaces, acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under this section. a) On or about the week of August 21st through 25th, 2017 and times prior thereto, located at Naval Weapons Station Cheatham Annex; C Street Warehouse #12, Williamsburg, VA 23185, Condition: Employee(s) entered a 5 x 15 x 15 foot underground storage tank confined space to clean and remove interior sediment without being previously trained in the knowledge and skills necessary for the safe performance of the duties assigned. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
