29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) On or about September 09, 2017 - at the above addressed jobsite- an employee installing aluminum siding was working on a scaffold platform attached to the forks of a forklift/PIV without the use of personal fall protection, thereby exposing the employee to fall hazards of approximately thirty (30) to thirty five (35) feet.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedures to control or minimize those hazards:  a) On or about September 09, 2017 - at the above addressed jobsite - an employee installing aluminum siding was not provided proper training to recognized fall hazards associated in the work environment. The employee was working on a scaffold platform attached to the forks of a forklift/PIV without the use of personal fall protection, thereby exposing the employee to fall hazards approximately thirty (30) to thirty five (35) feet.

29 CFR 1926.1053(b)(22): Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  a) On or about September 11, 2017 - at the above addressed jobsite , an employee was using a ladder to access the work level, while carrying siding materials that could cause the employee to lose balance and fall approximately fifteen (15) to eighteen (18) feet.
