29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: a) On September 11, 2017, at the McLane Ohio facility in Findlay, Ohio: The employer did not ensure that employees were provided with adequate training on electrical safety-related work practices such as, but not limited to, the use of personal protective equipment to protect against electrical hazards while working on or near energized or potentially energized electrical parts. Employees were exposed electrical hazards while verifying power on a 280 volt circuit on Crane #4.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: a) On September 11, 2017, at the McLane Ohio facility in Findlay, Ohio: the employer did not ensure that employees verifying energy on a 280 volt system on Crane #4 were protected from electrical shock and arc flash hazards. Employees were not wearing personal protective equipment such as, but not limited to, hand protection while performing this task.
