29 CFR 1926.454(b): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question:  On or about September 12th, 2017, an employee operating the JLG Boom Lift was exposed to falling 15 feet to the ground below and the employer had not provided training to the employees on the nature of scaffold hazards, correct procedures for operating the scaffold, the design criteria and the intended use of the scaffold.
