29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about 9/12/2017, located at State & Pleasant Valley Plaza in Parma, OH 44129, the employer failed to ensure that employees were provided, and utilized fall protection while conducting roofing tear-off activities at a height of about 15 feet, with a slope of 5/12.
