29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) 4448 Aubrey Ave, Philadelphia - On or about 5/17/17, employees accessing a residential roof climbed a 32ft extension ladder that did not extend at least 3 ft. above the roof landing, exposing employees to fall hazards.  Vivint Solar, Inc. (dba Vivint Solar Developer, LLC) was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 898846, citation number 1, item number 2, and was affirmed as a final order on 6/10/2013, with respect to a workplace located at 16 Chapin Rd, Pine Brook, NJ  07058.  Vivint Solar, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 758441, citation number 1, item number 2, and was affirmed as a final order on 4/8/2013, with respect to a workplace located at1 Doric Ave, Parsippany, NJ  07054

(a) 4448 Aubrey Ave, Philadelphia - On or about 5/17/17, employees using a 32ft extension ladder was not used at an angle of approximately one-quarter of it's height, exposing employees to fall hazards.  Vivint Solar Developer, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 988131, citation number 1, item number 1, and was affirmed as a final order on 11/4/14, with respect to a workplace located at 79 Cheever Ave, Dracut, MA.
