Pugmill at unit 5 ash unloader: On or about 13 September 2017, Machine guarding was not provided to protect operators from laceration, amputation, and caught-in hazards by the dual rotating auger mixing mechanism (pugmill)where operators control water actuator to combine water with fly ash in order to expel it from the building into an open top box truck.
