29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: a) East side of home, front, on or about 8-13-17: Employees were using a platform material hoist as a ladder and exiting by jumping over the platform to the ground, a change in elevation of 27 up to 37 inches.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) South side of garage, on or about 9-13-17: Five employees used a portable wood extension ladder for access to a prefabricated aluminum scaffold platform, with side rails that were 28 inches above the landing surface, exposing the employees to a fall hazard of 9 feet 2 inches.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) West side of roof, north end, on or about 9-13-17: Five employees were working on a sloped roof (6:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of up to 18 feet 2 inches to the ground below. b) West side of roof, center, on or about 9-13-17: Five employees were working on a sloped roof (6:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of 16 feet to the wood deck below. c) South side of home, garage, on or about 9-13-17: Five employees were working on a sloped roof (8:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of 10 feet 1 inch to the ground below. d) West side of roof, south end, on or about 9-13-17: Five employees were working on a sloped roof (6:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of 9 feet 2 inches to the sunroom and/or garage roof below.
