29 CFR  1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.   a) On or about 01/26/2025:  The employer did not provide abatement documentation for Citation 02, Item 002which became a final order on April 2, 2024.  An abatement demand letter was sent to the employer on February 4, 2025.  Abatement documentation is required for this item.

29 CFR  1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).    a) 201 West Malloy Road, Syracuse, NY 13211 - On the roof.  On or about 9/15/23, employees were using a portable extension ladder to access the roof.  The ladder was at an approximate angle of 45 degrees (1:1).  Employees were exposed to falls up to 9.5 feet.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) 201 West Malloy Road, Syracuse, NY 13211 - On the roof.  On or about 9/15/23, employees were exposed to falls of up to 20 feet while performing roofing work on an 8:12 section of roof feet without fall protection.  b) 201 West Malloy Road, Syracuse, NY 13211 - On the roof.  On or about 9/15/23, an employee was exposed to falls while removing a brick chimney as part of the roofing work, on a 6:12 section of roof with a ground to eave height of 14 feet without fall protection.     Charles Pisik was previously cited for a violation of this occupational safety and health standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 1415430, citation number 2, item number 1 and was affirmed as a final order on 10/4/19 , with respect to a workplace located at 1153 State Street, Watertown, NY  13601.

On or about 9/15/23, employees were exposed to falls while performing roofing work on an 8:12 section of roof with a ground to eave height of up to 14 feet without fall protection and without having been trained on the use of fall protection equipment or hazards related to falls in construction.  Charles Pisik was previously cited for a violation of this occupational safety and health standard CFR 1926.503(a)(1), which was contained in OSHA inspection number 1415430, citation number 1, item number 4 and was affirmed as a final order on 10/4/19 , with respect to a workplace located at 1153 State Street, Watertown, NY  13601.

On or about 9/15/23, employees were exposed to falls while using an extension ladder to access the upper roof (dormer) where that ladder did not extend at least 3 feet above the upper landing.   Charles Pisik was previously cited for a violation of this occupational safety and health standard CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1415430, citation number 3, item number 2 and was affirmed as a final order on 10/4/19 , with respect to a workplace located at 1153 State Street, Watertown, NY  13601.
