29 CFR  1926.416(a)(1): The employer did not prevent employee(s) from working in proximity to an electric power circuit that the employee could contact during the course of work and the employee(s) were not protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means:  a) On or about 10/18/2023 on the site located at 323 Bailey Road, Syracuse, NY, 13212: Employees were not protected from electrical shock when using multiple tools, including but not limited to pneumatic nail guns, hammers and a scrapers in close proximity to incoming 240-Volt service entrance.

29 CFR  1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed.   a) On or about 10/18/2023, on the site located at 323 Bailey Road, Syracuse, NY 13212: An employee was using a ladder horizontally strapped to a truck to stand on and to access the roof of the residential home.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b):  a) On or about 10/18/2023, on the site located at 323 Bailey Road, Syracuse, NY 13212: An employee working on a 5:12 residential structure without utilizing fall protection was exposed to a fall distance of approximately 16 feet.  Charles Pisik DBA Cross Country Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1415430, citation number 2, item number 1 and was affirmed as a final order on 11/08/2019, with respect to a workplace located at 1153 State Street, Watertown, NY 13601.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  a) On or about 10/18/2023, on the site located at 323 Bailey Road, Syracuse, NY 13212: Employees were using a ladder for roof access that was not extended 3 feet above the upper landing surface were exposed to a fall distance of approximately 10 feet.  Charles Pisik DBA Cross Country Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.1053(b)(1), which was contained in OSHA inspection number 1415430, citation number 3, item number 2 and was affirmed as a final order on 11/08/2019, with respect to a workplace located at 1153 State Street, Watertown, NY 13601.
