29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two: On or about September 13, 2017 an employee was exposed to a fall hazard while working from a scaffold platform and the employer did not ensure that the horizontal lifeline, in use, was designed, installed and used under the supervision of a qualified person.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about September 13, 2017, an employee performing tuck-pointing activities from a scaffold platform was exposed to a fall hazard and the employer did not ensure that appropriate fall protection was utilized. Garneau Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 1093871, citation number 1, item number 3 and was affirmed as a final order on December 8, 2015, with respect to a workplace located at 530 N. Center St., Bloomington, IL 61701.
