29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:    a) 75 Overbrook Avenue, Edison, NJ 08817. Employee was exposed to fall hazards in excess of 22 feet from the eave to the ground below while installing new roofing materials with no fall protection system in place.     Occurred on 09/14/2017

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:  a) 75 Overbrook Avenue, Edison, NJ 08817. Employee was exposed to fall hazards in excess of 22 feet from the eave to the ground below when using a portable aluminum extension ladder to access the roof when the ladder did not extend three feet above the landing.   Occurred on 09/14/2017
