29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  a) Jobsite - The employer did not ensure that each working level was fully planked or decked while employees were performing bricklaying operations, exposing the employees to fall hazards of approximately twenty-four (24) feet.  Observed on or about 16 AUG 2017.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph.  a) Jobsite - The employer permitted the construction of a tubular welded scaffold to be built without guying, tying or bracing.  The scaffold was built with a total height of approximately twenty-eight (28) feet and a width of approximately five (5) feet resulting in scaffold height to width ratio of 5.6:1.  Observed on or about 16 AUG 2017.

29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).Note:  Specify distances as indicated in table in paragraph (f)(6) of this section.  a) Jobsite - The employer did not maintain at least three feet between the tubular welded scaffold and the power lines when the employer mounted the electrical service drop to the metal frame of the scaffolding.  Observed on or about 16 AUG 2017.
