29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units:  (a). Exterior 214 Main Street South-East Corner- Unstable objects were used to support a platform used by employees to access the building. Employees were accessing a building by walking up planking that was supported by a platform composed of hollow concrete blocks and wooden pallets, on or about September 13, 2017.  There is no abatement certification or documentation required for this item.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    (a). Interior 210 Main Street West Side-  Employees on the first floor walking/working surface were not protected from falling through a hole leading to the basement by the use of personal fall arrest systems, covers, or guardrail systems erected around the hole. Employees working in the area were using the adjacent stairway to access the upper levels where additional work was being performed, on or about September 13, 2017.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Employees working in the area were using the adjacent stairway to access the upper levels where additional work was being performed, on or about September 13, 2017.
