29 CFR 1910.22(a)(2): For a wet process, drainage was not maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats was not provided: (a) Tent Washing Area: On or about September 13, 2017, an employee was working in standing water in the area of the tent washing area. Some of this water was from the operation of tent washing and some of this standing water was from leaks in the roof. There were no raised areas (mats, platforms, etc.) for employees to stand to get out of the water.

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: (a) Facility: On or about September 13, 207, the TCM Pro-6 powered industrial truck (forklift) had a yellow running light that was not working. Also, the Hyster 50 powered industrial truck (forklift) had a front left light that was not working along with the back lights that do not work. In addition, the Heli powered industrial truck (forklift) had it back right light not working.
