The employers working below were exposed to stuck-by hazards while the roofing material was thrown from the roof level.       b) Quality Multi Services LLC on September 21, 2017 at the address of 3141 Manley Road, Maumee, OH: The employer failed to utilize an enclosed chute, directing waste material into the disposal container nearly 22 feet below the roof eaves from a scaffold platform.
