29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated  Worksite: The employees who were required to operate and work from the aerial lift basket were neither provided with nor wearing a personal restraint system that could be attached to the basket or boom of the aerial lift.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.  Worksite: The employees who were required to operate and work from the aerial lift basket were not trained to recognize the hazards associated with the type of scaffold being used.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  Worksite: The employer did not ensure that the employees who were working on the roof of a commercial building were protected from fall hazards above 6 feet by the use of guardrails, safety nets, personal fall arrest systems or a monitoring system.
