29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:  On or about September 15, 2017, an unqualified employee was exposed to electrical shock and burns while performing maintenance activities from an aerial lift and the employer had not ensured that the employee was trained in the requirements of 29 CFR 1910.333(c)(3)(i)(A).

29 CFR 1910.333(c)(3)(i)(A)(1): When an unqualified person was working in an elevated position near overhead lines with voltages to ground rated at 50 kV or below, the location was not such that the person and the longest conductive object he or she could contact came closer than 10 ft. (305 cm) to any unguarded, energized overhead line:  On or about September 15, 2017, employees working in an aerial lift near overhead power lines were exposed to electrical hazards and the employer did not ensure that the employees maintained a distance of at least 10 feet from the power lines.
