29 CFR 1910.261(l)(2): Emergency stops. Electrically or manually operated quick power disconnecting devices, interlocked with braking action, shall be provided on all operating sides of the machine within easy reach of all employees. These devices shall be tested by making use of them when stopping the machine. Machine 1 winder, Seaman Paper, 51 Main St., Otter River, Massachusetts; On and before September 28, 2017 the winder for machine 1 was operated with only a single emergency stop switch located at the control panel. The machine has multiple operators and some are not visible from the control station during operation and far out of reach of the emergency stop switch.

The monorail, underhung cranes at machines 1 and 2: On or about September 18, 2017, the monorail underhung cranes were not inspected as required. Specifically the chains and hooks on the cranes were not inspected to check for stretching of the chain or the throat of the hooks for deformities.

Specifically the cranes do not have bumpers, trolley stops, or rail sweeps.
