OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to guarding hazards: a) At the maintenance shop, on or about May 2017: The employer permitted an employee to operate a Jet Drill Press without a guard around the rotating chuck, exposing the worker to an amputation hazard. Amongst other feasible means of abatement: Install an adjustable guard around the rotating chuck to comply with the American National Standards Institute, ANSI B11.8-2001, Safety Requirements for Manual Milling, Drilling and Boring Machines with or without Automatic Control, per Figure A.4 or A.5.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire hazards. a) At the maintenance shop, on or about 9/18/2017: Carts containing oxygen and acetylene tanks were kept together when in storage, exposing employees to fire hazards. Amongst other feasible means of abatement: Ensure oxygen cylinders in storage are separated from fuel-gas cylinders or combustible materials, a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour to comply with the Compressed Gas Association (CGA), P-1-1965, Section 3.5.3
