29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. 107 Fowler Street, New Haven, CT: On/about August 24, 2017, an employee suffered an amputation of the fifth (5th) digit ("pinky or little finger") on the right hand while closing a gate and the incident was did not reported to OSHA within twenty-four (24) hours.
