29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by wearing protective helmets:  a) Roof - Employees were not provided with head protection while operating pneumatic nail guns to attach shingles to a roof and were exposed to ricochet nails, on or about September 18, 2017.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  Upper Main Roof - Employees conducting residential roofing operations on a steep roof were not provided with fall protection and were exposed to an approximate 22 feet fall hazard to the ground below, on or about September 18, 2017.  b)  Lower Garage Roof - Employees conducting residential roofing operations on a steep roof were not provided with fall protection and were exposed to an approximate 13 foot fall hazard to the ground below, on or about September 18, 2017.   Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridgid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the ladder:  a)  Upper Main Roof -  Employees used a 24 foot portable extension ladder to access the main roof and the side rails could not extend 3 feet above the landing surface, the ladder was not secured at the top, and a grabrail was not provided to assist in mounting and dismounting the ladder, on or about September 18, 2017.  Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
