29 CFR 1910.178(h)(2): Where general lighting was less than 2 lumens per square foot, auxiliary directional lighting was not provided on the truck: A. On or about September 20, 2017, Warehouse: The auxiliary lighting on the company's Toyota propane-powered industrial truck was not operational. No dock lights were installed at dock doors #1, #2, #4 or #5. The dock light at dock #6 did not have a bulb. Employees operated the forklift at the docks in the early morning, entering trailers in the dark.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and/or cables were run through holes in walls, ceilings, or floors: A. On or about September 20, 2017, Warehouse, near Office: A metal-encased Central Machinery 32-inch diameter pedestal fan was powered through a flexible extension cord. The extension cord was run underneath dollies in storage, around a metal support beam, up the wall and through a pass-through window to the office area. In the office, the extension cord ran over a table, and across the floor, powered through a wall outlet underneath the table.
