29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. a) Facility, 2053 S.R. 209 Brodheadsville, PA - Employees operated a Mitsubishi Caterpillar Forklift LP Model 25N (Serial No. C-459109) with an attached Forklift Boom Model AFJ-25 (Serial No. 0200812002) to lift stone slabs on a daily basis. The forklift lacked allowable attachment markings which exposed employees to fall hazards and struck-by hazards due to the unknown modifications to the forklift's lifting capacity and center of gravity created by the use of a front-end attachment, on or about September 19, 2017. In accordance with 29 CFR 1903.19, abatement certification is required for this violation.

In accordance with 29 CFR 1903.19, abatement certification is not required for the violation. The hazardous condition was corrected during the inspection.
