29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval.    a) Facility, 2053 S.R. 209 Brodheadsville, PA - Employees operated the Mitsubishi Caterpillar Forklift LP Model 25N (Serial No. C-459109) with an front-end attached Abaco Forklift Boom Model AFJ-25 (Serial No. 0200812002) without the manufacturer's prior written approval exposing employees to struck-by hazards created by the modifications to the powered industrial truck's lifting capacity and center of gravity, on or about September 19, 2017.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.    a) Facility, 2053 S.R. 209 Brodheadsville, PA - Employees operated a Mitsubishi Caterpillar Forklift LP Model 25N (Serial No. C-459109) with an attached Forklift Boom Model AFJ-25 (Serial No. 0200812002) to lift stone slabs on a daily basis. The forklift lacked allowable attachment markings which exposed employees to fall hazards and struck-by hazards due to the unknown modifications to the forklift's lifting capacity and center of gravity created by the use of a front-end attachment, on or about September 19, 2017.    In accordance with 29 CFR 1903.19, abatement certification is required for this violation.

The forklift's nameplate had an illegible load rating chart exposing employees to fall hazards and struck-by hazards due to unknown load capacities, on or about September 19, 2017. 29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check:  a) Grinding/Polishing Room, 2053 S.R. 209, Brodheadsville, PA - Two (2) Amerex 10 LB Multi-Purpose Dry Chemical Fire Extinguishers had not received an annual maintenance check since August 2012 (one (1) of which was partially charged) and employees who attempted to extinguish a fire with the portable fire extinguisher were exposed to fire hazards, on or about September 19, 2017.  In accordance with 29 CFR 1903.19, abatement certification is not required for the violation. The hazardous condition was corrected during the inspection.

29 CFR 1910.305(c)(4): Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface.   a) South Wall, Grinding/Polishing Room, 2053 Route 209, Brodheadsville, PA - The flush-mounted snap switch (light switch) that operated fluorescent lights had energized conductors that were uncovered due to a missing faceplate, exposing employees to a contact with electrical current hazard, as observed on September 19, 2017.  In accordance with 29 CFR 1903.19, abatement certification is not required for the violation. The hazardous condition was corrected during the inspection.
