29 CFR 1910.138(a): The employer did not select and require employees to use appropriate hand protection when employees hands were exposed to hazardous condition(s) Shop; On or about 9/20/17, employees were not required to wear gloves when handling a nickel alloy being machined and sanded in the shop. Nickel is a skin sensitizer.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: Shop; On or about 9/20/17, employees were not trained to the hazards of the materials they were working with in the shop. The employees machine and hand sand copper alloys containing beryllium and nickel. The beryllium can cause chronic beryllium disease and cancer. The nickel is a skin sensitizer and may cause respiratory effects with chronic exposure. The welder welds stainless steel and has not been trained in the hazards of hexavalent chromium. Hexavalent chromium can cause cancer and irritation to the nose, throat and respiratory tract.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: Shop; On or about 9/26/18, the employer failed to report a partial amputation of the tip of his middle finger of an employee working on a grinding wheel. The employer's failure to report this prevented OSHA from performing an investigation of the incident.
