29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s). On or about September 20, 2017, the facility forklifts being use in the warehouse were observed need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition. This condition exposed employees to struck-by hazards in the following instances; a) Doosan-Daewoo (Model No. G30S) had the LPG container straps broken and tires damaged. b) Hyster (unknown Model) had the LPG container straps broken and backup alarm was not operational and missing name plate.. c) Nissan (Model No. MUGLO2A30LV) backup alarm was not operational. d) Mitsubishi (Model no. FGC25) backup alarm was not operational.
