29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed.  (a) On or about September 19, 2017 and at times prior, the employer has a Lockout/Tagout program with an effective date of September 20, 2012 and has not done an annual review since September 28, 2015.  Equipment requiring lock out tag out procedures were listed as the Propane Dispenser, Grain Bin and unloading system--syrup tank, Salt Lake elevator grain storage, Fanner system-Salt Lake elevator, BE 1506--mixing elevator leg, DC 1057--screen conveyor on mixer surge hopper, bagger system and sewing machine, barley cooler system, corn cooler system, barley steam flaker, corn steam flaker, barley reclaim system, corn reclaim system, commodity mixer, commodity receiving system, ingredient bin, receiving system, tallow tank molasses tank, complete feed mill/plant, Grain storage reclaim, Grain receiving system.  Abatement certification IS required for this item.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)  Cyclone Area: On or about September 19, 2017, and at times prior thereto, the employer did not have an auger sufficiently guarded; the auger measured 27 inches from the ground level with an opening approximately 20 x 16 inches exposing employees to the rotating auger.
