29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.  On or about September 21, 2017, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

On or about September 21, 2017, at the above addressed jobsite, employees performing residential roofing operations on a roof, eight (8) feet nine (9) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.
