29 CFR 1910.215(c)(1): Abrasive wheel(s) used on grinding machinery were not mounted between flanges which were less than one third the diameter of the wheel:    On or about 12/15/2017, the inside diameter (ID) grinder located in the grinding department was not equipped with flanges.  Employees tasked with completing finishing activities on custom/specialty ordered springs were potentially exposed to hazards created by the unguarded projection bit/stud of the abrasive wheel machinery.

29 CFR 1910.215(c)(1)(ii): Type 1 cutting-off wheels were not mounted between properly relieved flanges which have matching bearing surfaces. Such flanges were not at least one-fourth the wheel diameter:   On or about 12/15/2017, the company's cut-off wheel was inadequately guarded in that it was not equipped with proper flanges.  Employees who use the cut-off wheel during the production of (heavy gauge) metal springs were potentially exposed to struck-by hazards created from flying debris if the abrasive wheel were to explode/shatter and/or disintegrate and become a projectile.

The box was not mounted to a fixed surface.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment:  On or about 12/15/2017, the employer failed to ensure that a junction box used as a power source by employees who operated the company's pedestal grinder was free from damage. The housing was missing a cover (knock out) and thus potential exposed the operator(s) to electrical hazards from the wiring contained within the unit. Energy sources used at the facility include but are not limited to: Electric -110, 220 volt single phase, 220 - 440/480 volt 3 phase and pneumatic energy.
