29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Employees were exposed to amputation hazards from mechanical power presses. Lockout tagout was not being utilized when employees are cleaning powder from the mechanical power presses, such as but not limited to Molder # 215. On or about September 14, 2017 an employee received an amputation injury when cleaning power out of the Molder # 215 when it was still running.

29 CFR 1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s): a) Employees were exposed to amputation hazards from Mechanical Power Presses, such as, but not limited to the M-215 Eastern Part Revolution Mechanical Power Press in South Molding. The guard did not prevent entry of hands or fingers in to the point of operation. The guard at the front of the press had openings where employees could reach over the guard and under the guard into the point of operation. On or about September 14, an employee was injured when the employee reached under the guard and received a partial finger amputation. b) Employees were exposed to amputation hazards from the M-217 Hasting Press. The M-217 Hasting Part Revolution 15 ton press is located in South Molding. Employees were exposed to amputation hazards as the guard on the back of the press was missing. c) Employee were exposed to amputation hazards from the M-217 Hasting Press. The M-217 Hasting Part Revolution 15 ton press is located in South Molding. The guard at the front of the press had openings where employees could reach over the guard and under the guard to the point of operation. d) Employees were exposed to amputation hazards from the M-218 Eastern Press. The M-218 Eastern Part Revolution Mechanical Power Press in located in South Molding. The guard at the front of the press did not prevent employees from reaching over and under the guard to the point of operation.
