The companys owner stated that they never filled out such forms since he purchased the company 6 years ago.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: On or about 09/20/2017, Midwest Drywall Professionals Inc., located at 7503 Mexico Road, St. Peters, MO, stated that they did not have a competent scaffold person inspect and release their scaffold for work. The scaffold was located at The Barton, 8500 Maryland Ave., Clayton, MO, in the elevator shaft in Building #2.
