29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets.    On or about September 21, 2017, employees were remodeling the building under construction from an aerial lift approximately 30 feet high without hard hats while another employee was working directly above, exposing employees to the hazard of being struck by tools or materials.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    On or about September 21, 2017, an employee was remodeling the building under construction from an aerial lift approximately 30 feet high using a reciprocating saw without the use of safety glasses, exposing the employee to the hazard of being struck in the eye by flying objects or materials.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated    On or about September 21, 2017, employees were remodeling the building under construction from an aerial lift approximately 30 feet high without fall protection, exposing employees to a fall hazard.
