29 CFR 1910.176(a): Aisle(s) or passageway(s) were not kept in good repair: A) National Industrial Lumber Company (NILCO), located at 2803 Toledo Avenue, Lorain, OH 44055: On or about September 13, 2017, the employer did not ensure that passageways, used by forklift operators to move material in the facility, were kept in good repair. There were potholes in the facilitys yard, resulting in uneven surfaces and exposing employees to struck-by and crushing hazards in that forklift operators dropped material from the forklifts when driving over the potholes.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): A) National Industrial Lumber Company (NILCO), located at 2803 Toledo Avenue, Lorain, OH 44055: On or about September 20, 2017, the employer did not provide training and evaluation to ensure that employees, operating powered industrial vehicles such as, but not limited to, the Mitsubishi forklift (Model FG45N, Serial # AF29D50034) and the Caterpillar forklift (Model GP45N1, Serial # AT29D60227) were competent to safely complete the task. Employees were exposed to struck-by, caught-in-between, and tip-over hazards.

Employees were exposed to struck-by and caught-in-between hazards.
