29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section: On or about September 22, 2017, the employer failed to ensure that procedures for the control of hazardous energy were developed, documented and/or used by employees tasked with sanitizing machinery such as; but not limited to, the proofer machinery on line number #4. Energy sources used at the facility include; but are not limited to, electrical and pneumatic energy.

29 CFR 1910.147(e)(2)(i): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not ensure that all employees had been safely positioned or removed from the work area: On or about September 16, 2017, employees tasked with sanitizing line #4 (the intermediate proofer) were not protected from hazards created by unexpected start-up of hazardous energy sources in that the company did not develop a policy or procedure to ensure that prior to removal of any/all lockout devices, all employees in the immediate work area were safely positioned or removed from the hazardous area.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: On or about September 16, 2017, group lockout was not being utilized by employees as they sanitized line #4 (the intermediate proofer), and thus the employees were exposed to hazards created by unexpected start-up of hazardous energy sources. Orlando Baking Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR. 1910.147 (f)(3)(ii)(D), which was contained in OSHA inspection number 957328, citation number 01, item number 003c and was affirmed as a final order on June 10, 2014 with respect to a workplace located at 7777 Grand Ave., Cleveland, OH 44104.
