29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about September 22, 2017 one employee was observed using a pneumatic nail gun to secure plywood sheathing to the roof trusses without wearing any eye protection.  The same employee was observed using a DeWalt brand circular saw to cut the plywood to size without wearing any eye protection. The employee was exposed to impact eye injuries from debris and saw dust because he was not protected by appropriate eye or face protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about September 22, 2017 one employee was observed working on the roof of a single story residential dwelling with a 5:12 pitch, steep slope roof installing roof sheathing and truss blocking and was not protected from falling by any means of personal fall protection, guardrails, or safety nets, exposing him to a fall of more than 9 feet to the ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a) On or about September 22, 2017 the employees were using an 8 foot Werner brand fiberglass step ladder, in the closed position, leaning against the exterior concrete wall to gain access to the roof trusses to climb up and access the roof.

On or about September 22, 2017 one employee had used an 8 foot Werner brand stepladder, to gain access to the roof surface, 10 feet above the ground. The employee used the top step as a step in order to climb up onto and through the roof trusses.
