Located at 5055 S. Lemay Ave. Fort Collins, Co 80525: The employer did not ensure that the point of operation on a Globe Chefmate Model # GC12 fool slicer was guarded to prevent employees from having their hands in the danger zone during operation. Employees used the end weight handle in place of the missing food chute handle to operate the slicer. This exposed employees to amputation hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: a) Healthcare Services Group Inc. Located at 5055 S. Lemay Ave, Fort Collins, CO 80525: The employer did not ensure that the flexible cord for the Chefmate GC12 Meat Slicer was connected to its plug so that tension would not be transmitted to joints or terminal screws. The cord's outer jacket of insulation was separated near the plug, and exposed the inner conductors to the lack of strain relief. This condition exposed the employees to electrical hazards.

Located at 5055 S. Lemay Ave, Fort Collins, CO 80525: The employer did not ensure that the power cord for the Chefmate GC12 Meat Slicer was visually inspected prior to use for external defects. The outer insulation was damaged and failed to protect the internal wiring. This condition exposed the employees to electrical hazards.
