This condition exposed employees to fall hazards of more than 6-ft. to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Construction Site: On or about September 22, 2017, two employees were observed working on a roof without any form of fall protection equipment. The employer did not provide a training program to enable employees to recognize the hazards of falling. This condition exposed employees to fall hazards of more than 6-ft. to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  Construction Site: On or about September 22, 2016; two employees were using a 17-foot combination/extension ladder that was not positioned 36 inches above the did not extend at least 3-feet above the upper landing surface.
