29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: Instances including, but not limited to, the following: a) Job site, 114 Riviera Drive, West View, PA: On or about August 1, 2017, during roof sheathing activities, the employer did not ensure that an employee used appropriate fall protection systems/devices, where exposed to a fall hazard of up to approximately 20 feet. b) Job site, 114 Riviera Drive, West View, PA: On or about August 1, 2017, during roof sheathing activities, the employer did not ensure that there was either a cover put over a hole in the roof or that guardrails were put up around the hole, exposing an employee to a fall hazard of up to approximately 10 feet. c) Job site, 114 Riviera Drive, West View, PA: On or about August 1, 2017, during construction activities, the employer did not ensure that an employee used appropriate fall protection systems/devices when working on a landing for a stairway system, exposing an employee to a fall hazard of up to approximately 10 feet.

29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided: a) Job site, 114 Riviera Drive, West View, PA: On or about August 1, 2017, during construction activities, the employer did not ensure that a safe way to get up/down between floors was provided. Employees were permitted to climb wall stud framing to get up/down from the 2nd floor down to a stairway landing platform and from this platform to the 1st floor.
